Open your image in PSP.

Image > Resize >
Pixel Size height & width 200
All layers Checked
Maintain aspect ratio UNchecked.

Effects > Plug-in Filters > VM Distortion > Flip Chop > Default settings

Effects > Plug-in Filters > VM Distortion > Flipper > Default settings

Effects > Plug-in Filters > Eye Candy 3 > Weave > these settings:
Ribbon Width 30
Gap Width 2
Fill Gaps highlighted
Gap Color is your choice ( I used black)
Shadow depth 50
Thread detail 50
Thread length 8
Smear threads UNhighlighted

Effects > Plug-in Filters > Simple > Pizza Slice Mirror
