Shift + D to copy your image, then close the original.

In the Layer Palette right click on the background layer and Promote to layer.

Right click again and duplicate the layer.

Now you need to decide if you want to do the full color version or the grey scaled version.

If you want to make the grey scaled version:

Image > Greyscale.

Image > Increase Color Depth > 16 million colors.

Now the rest of the steps are the same for both versions.

Activate the top layer.

Layers > New Mask Layer > From image > these settings:
Drop down choose your mask image
Source luminance ticked
Invert UNchecked

Layers > Delete.
The pop-up will ask if you want the mask merged, click yes.

Layers > Merge > Merge group.

Effects > 3D effects > Drop shadow > these settings:
Horizontal & Vertical 5
Opacity 50
Blur 5
Color black

Effects > 3D effects > Drop shadow > these settings:
Horizontal & Vertical -5
Opacity 50
Blur 5
Color black

Adjust > Hue and Saturation > Colorize > these settings:
Hue 12
Saturation 50
(More or less depending on your liking.)

That's all there is to it!
